For postal entries, download and print 1 or more entry form(s) and send it/them completed before November 16, 2010, along with your songs on audio CD(s), data CD and/or data DVDs, to:

We can also send entry forms by e-mail along with the Rules and Regulations of the contest. You can request them via the contact page  by choosing the 'Entry Form Request' option if you would like to receive them by mail, and the 'Entry Information' option if you would to receive them by e-mail.
